In order to vote, you must agree to the following Solemn Declaration:  “I solemnly declare that: I am a member of this congregation of the Anglican Church of Canada; I am of the full age of sixteen years; I am a regular worshipper with this congregation; I do not intend to vote as a member of any other Vestry in any other Anglican Parish in the Anglican Diocese of Ottawa during the ensuing year; and I am a supporter of this congregation by regular contributions to its maintenance.”

If you agreed to the above Solemn Declaration, please type out your name below. If there will be multiple people joining through the same device (e.g. a family gathered around a single computer) please enter the name of all those who agree with the Solemn Declaration. If it turns out that they are no longer able to attend no further action needs to be taken. If they no longer wish to participate on the same device and prefer to use a separate one, or if there are people that wish to join you on the same device after you submit this form, they must fill out this form for themselves. They will get a Zoom link, but they do not need to use it. Each person who agrees to the Solemn Declaration will still get a vote, even if they are on the same device. Instructions on voting will be given during the Vestry meeting.
